APPLICATION DOCUMENTS
(1) A copy of the bachelor's degree from the faculty of science or engineering of the person who will serve as the radiation protection officer (RPO) within the organization and a copy of the document proving that he has been successful in the exam on "Radiation Protection in Industrial Applications (RPIA)" (Annotation I)
(2) Emergency Plan
(3) Sketch showing the layout of the device and the purpose for which adjacent areas (bottom, top and side) are used
(4) Applicant for a licence;
a) If it is a real person, a copy of the tax certificate
b) If it is a private company, a copy of the current trade registry gazette and tax certificate
c) If it is an association or foundation, the document showing the parent company
(5) Original license certificate
(6) A copy of the document received from the municipality regarding the change of the address
(7) Petition stating the name, full address and necessary contact numbers of the organization where the radiation source is sold or handed over.
(8) A petition stating the full address of the place where the x-ray device, which does not contain radioactive source, is stored, and that a license application will be made in case the device is planned to be reused,
(9) A copy of the report showing that the x-ray device that does not contain a radioactive source is scrapped
(10) A copy of the document (bill of lading, etc.) showing that the radioactive source or the device containing the radioactive source has been sent to its origin.
