SCOPE:
- Practices using unsealed radioactive sources in the laboratory, Mobile x-ray explosive detection devices
- Calibration and research practices using sealed radioactive sources in the laboratory
- Sealed radioactive sources used in device detector calibration
- Cs-137 guide sources used in radiography practices (license fee must be paid for each guide source)
- Blanket type (Th-232, K-40, U-238) calibration sources used in well logging practices ( license fee must be paid for each source)

APPLICATION DOCUMENTS:
(1) Radiation Protection Officer (RPO);
a) Copy of at least 4-year undergraduate diploma in related subjects
b) for Cs-137 guide sources used in radiography, required documents for RPO in radiography applications
(2) Sketch showing the layout of the laboratory (or the room where resources will be stored safely when not in use) and the purpose for which adjacent areas (bottom, top and side) are used
(3) Applicant for a license;
a) If a real person, a copy of the tax certificate
b) If it is a private company, a copy of the current trade registry gazette and tax certificate
c) If the association and foundation, the document showing the parent organization
(4) Original license certificate
(5) A copy of the document received from the municipality regarding the change of the address
(6) Petition stating the name, full address and necessary contact numbers of the organization where the laboratory or the resource(s) used in the laboratory is sold or transferred
(7) Document (bill of lading, etc.) showing that the radioactive source(s) has been sent to its origin.
(8) Copy of radioactive waste delivery report issued by TENMRA Nuclear Energy Research Institute radioactive waste processing and storage unit
