APPLICATION DOCUMENTS:




(1) Radiation Protection Officer (RPO);

a) If he is a graduate of the faculty of science or engineering, a copy of the diploma and a copy of the certificate on 'Radiation Protection in Industrial Radiography' (Footnote I) showing that he/she was successful in the exam,

b) If there is a Level-3 certificate on radiography, which is one of the non-destructive testing methods, a copy of the certificate on 'Radiation Protection in Industrial Radiography' (Footnote I) showing that he/she was successful in the exam and a CV stating at least two years of experience in radiation practices

(2) Emergency Plan

(3) Sketch showing the layout of the device/resource (or the room where the device is kept safe when not in use) and what the adjacent areas (bottom, top and side) are used for

(4) Applicant for a license;

a) If a real person, a copy of the tax certificate

b) If it is a private company, a copy of the current trade registry gazette and tax certificate

c) If the association and foundation, the document showing the parent organization

(5) A petition stating the full name and address of the company providing the service, with a sample of the contract or invoice indicating that the tracking system service (GPS/GSM, etc.) is received for real-time tracking of mobile/portable radiography devices containing radioactive sources.

(6) Type B transport container certificate valid for radiography (gammagraphy) devices containing radioactive source (Footnote II)

(7) Original license certificate

(8) A copy of the document received from the municipality regarding the change of the address

(9) Petition stating the name, full address and necessary contact numbers of the organization where the radiation source is sold or transferred

(10) A petition stating the full address of the place where the radioactive source-free x-ray device is stored and that a license application will be made in case the device is planned to be reused.

(11) Copy of the report showing that the x-ray device that does not contain a radioactive source is scrapped.

(12) A copy of the document (bill of lading, etc.) showing that the radioactive source or the device containing the radioactive source has been sent to its origin

(13) Copy of radioactive waste delivery report issued by TENMRA Nuclear Energy Research Institute radioactive waste processing and storage unit

(II) In case the validity period of the type B transport container certificate expires, the license granted loses its validity. In this case, it is necessary to send the radiation source to its origin, and if it cannot be sent to its origin, the ÇNAEM-DDRAY unit should be delivered to the waste facility and an application for license cancellation should be made. For this reason, before the validity period of the B type transport container certificate expires, if the certificate has been updated, the current version should be sent to the Authority to ensure the continuation of the license validity.
