APPLICATION DOCUMENTS:




(1) Radiation Protection Officer (RPO);

c) If it is an active duty officer or non-commissioned officer, a copy of the document related to the training he received regarding the safe use of the device and a copy of the document proving that he was successful in the exam on "Radiation Protection in Industrial Applications (RPIA)" (Footnote I).

d) The Customs Enforcement officer, on the other hand, is a copy of the certificate regarding the training he received regarding the safe use of the device, and a copy of the certificate proving that he was successful in the exam on "Radiation Protection in Industrial Applications (RPIA)" (Footnote I).

e) A copy of a science or engineering diploma and a copy of the certificate proving that he has been successful in the exam on "Radiation Protection in Industrial Applications (RPIA)" (Footnote I)

(2) Emergency Plan

(3) Applicant for a license;

d) If a real person, a copy of the tax certificate

e) If it is a private company, a copy of the current trade registry gazette and tax certificate

f) If the association and foundation, the document showing the parent organization

(4) Original license certificate

(5) A copy of the document received from the municipality regarding the change of the address

(6) Petition stating the name, full address and necessary contact numbers of the organization where the radiation source is sold or transferred

(7) A petition stating the full address of the place where the radioactive source-free x-ray device is stored and that a license application will be made in case the device is planned to be reused

(8) Copy of the report showing that the x-ray device that does not contain a radioactive source is scrapped

(9) A copy of the document (bill of lading, etc.) showing that the radioactive source or the device containing the radioactive source has been sent to its origin

(10) Copy of radioactive waste delivery report issued by TENMRA Nuclear Energy Research Institute radioactive waste processing and storage unit
