APPLICATION DOCUMENTS
(1) Radiation protection officer (RPO);
a) If it is a nuclear medicine specialist, a copy of the certificate of expertise
b) If the person who has a certificate of authorization from the Ministry of Health to carry out studies using the RIA method, a copy of the document
(2) 1/100 scale architectural project or sketch showing the layout of the laboratory and the purpose of the adjacent areas (bottom, top and side) prepared according to the RIA Laboratory Specifications
(3) Those who apply for a license;
a) If it is a real person, a copy of the tax certificate
b) If it is a private company, a copy of the current trade registry gazette and tax certificate
c) If it is an association or foundation, the document showing the parent organization
(4) Original license certificate
(5) A copy of the document received from the municipality regarding the change of the address
(6) Petition stating the name, full address and necessary contact numbers of the organization where the laboratory or the device/devices used in the laboratory are sold or transferred
(7) A petition stating that the laboratory's activities will be stopped, the full address of the place where the used device/devices are stored, and that a license application will be made in case the laboratory is planned to be reused
(8) A copy of the report showing that the laboratory or the device/devices used in the laboratory are scrapped
