APPLICATION DOCUMENTS




(1) A copy of the training certificate received by the radiotherapy physicist who will be responsible for radiation protection, and a curriculum vitae showing his/her clinical experience. (Footnote I)

(2) Emergency Plan

(3) Information Required for Closed Source Applications (Footnote II)

(4) Showing of the location of the unit;

a) 1/100 scale approved architectural project prepared according to the Project Shieldding Report obtained from our Authority for the units with brachytherapy, linear accelerator, teletherapy and similar devices (Footnote III)

b) For operating theaters where closed source applications (prostate, eye, etc.) are made, and for units with simulator devices, the layout of the device, a sketch or architectural project showing the purpose of the adjacent areas (bottom, top and side)

(5) Quality Conformity Certificate for the devices to be used in the unit (Footnote IV)

(6) Those who apply for a license;

a) In case of a real person, a copy of the tax certificate

b) In case of a private company, a copy of the current trade registry gazette and tax certificate

c) In case of an association and foundation, the document showing the main organization

(7) For linear accelerator, cobalt-60, tomotherapy, gamma knife, brachytherapy or cyber knife devices, a letter of conformity obtained from the Ministry of Health General Directorate of Health Services, if it is a private company applying for a license, within the scope of device planning

(8) Original license certificate

(9) A copy of the document received from the municipality regarding the change of the address

(10) A Petition stating the name, the full address and the necessary contact numbers of the organization where the radiation source was sold or transferred.

(11) A petition stating the full address of the place where the x-ray device, which does not contain a radioactive source, is stored, and that a license application will be made in case the device is planned to be reused.

(12) A copy of the report showing that the x-ray device that does not contain a radioactive source was scrapped.

13) The document showing that the radioactive source or the device containing the radioactive source was sent to the origin (bill of lading, etc.)

(14) A copy of the radioactive waste delivery report issued by the Radioactive Waste Processing and Storage Unit of TENMRA Nuclear Energy Research Institute





FOOTNOTES:

(I) Those who have the title of physicist, physical engineer, health physicist or nuclear energy engineer and who have a science specialist, graduate engineer or doctorate degree by getting education from at least one of the science, health sciences or nuclear sciences institutes of the universities

a) Completed postgraduate education on the subject in one of the sciences/departments of nuclear physics, basic oncology, clinical oncology or radiation oncology, or

b) Those who have completed their postgraduate education outside of the above-mentioned sciences/departments, in a way to certify that they have actually worked in a radiotherapy facility licensed by our Authority and for at least one year in the field of radiotherapy (on quality assurance and radiation protection, including patient dosimetry, treatment planning, optimization, quality control). must submit an official document signed by the radiotherapy physicist and the facility manager.

(II) It is requested from operating rooms where closed source applications (prostate, eye, etc.) are made.

(III) Before the license application procedures, it is obligatory to send the unit project to the Authority and obtain an Shielding Conformity Report. Please refer to the 'Radiotherapy Project Application Documents' for obtaining the Shielding Conformity Report.

(IV) Not required for operating theaters where CT simulator and closed source applications (prostate, eye, etc.) are performed.
