APPLICATION DOCUMENTS




(1) Radiation protection officer (RPO);

a) In case of a dentist, a copy of the diploma

b) In case of a radiologist, a copy of the certificate of the expertise

c) A copy of the diploma if the radiology technician graduated from the Radiology Programme of the Health Vocational School

ç) A copy of the diploma if the radiotherapy technician graduated from the Radiotherapy Department of the Health Vocational School

d) In case of a dental technician and technician, a copy of the diploma and a copy of the certificate on "Radiation Protection in Diagnostic Radiology (RPDR)" (I) showing that he/she was successful in the exam.

e) In case of a physicist, physics engineer or nuclear energy engineer, a copy of the diploma and a copy of the certificate on "Radiation Protection in Diagnostic Radiology (RPDR)" (I) showing that he/she was successful in the exam.

f) In case of a radiology technician graduated from a health vocational high school, a copy of the diploma and a copy of the certificate on "Radiation Protection in Diagnostic Radiology (RPDR)" (I) showing that he/she was successful in the exam.

(2) 1/100 scale architectural project or sketch showing the layout situation of the room where the device will be used and the purpose for which the adjacent areas (bottom, top and side) are used. (click for the sample sketch) (Not required for mobile devices)

(3) Those who apply for a license;

a) In case of a real person, a copy of the tax certificate

b) In case of a private company, a copy of the current trade registry gazette and tax certificate

c) In case of an association and foundation, the document showing the main organization

(4) Original license certificate

(5) A copy of the document received from the municipality regarding the change of the address

(6) Petition stating the name, the full address and the necessary contact numbers of the organization where the device is was sold or transferred

(7) A petition stating that the full address of the place where the device is stored and that a license application will be made in case the device is planned to be used again.

(8) A copy of the report showing that it was scrapped
