APPLICATION DOCUMENTS




(1) Radiation protection officer (RPO);

a) In case of a radiologist, nuclear medicine specialist, radiation oncologist, radiotherapy physicist, a copy of the specialty / training certificate

b) In case of other physicians, a copy of the diploma and a copy of the certificate on "Radiation Protection in Diagnostic Radiology (RPDR)" (I) showing that he/she was successful in the exam (Footnote I)

c) In case of a graduate of science or engineering faculty, a copy of the diploma and a copy of the certificate on "Radiation Protection in Diagnostic Radiology (RPDR)" (I) showing that he/she was successful in the exam (Footnote I)

(2)The document showing the on-the-job training received from the authorized company, including the radiation safety training of the person to be RPO and the use of the blood irradiation device

(3) 1/100 scale architectural project or sketch showing the layout situation of the room where the device will be used and the purpose for which the adjacent areas (bottom, top and side) are used.

(4) Those who apply for a license;

a) In case of a real person, a copy of the tax certificate

b) In case of a private company, a copy of the current trade registry gazette and tax certificate

c) In case of an association and foundation, the document showing the main organization

(5) Original license certificate

(6) A copy of the document received from the municipality regarding the change of the address

(7) A Petition stating the name, the full address and the necessary contact numbers of the organization where the radiation source was sold or transferred.

(8) A petition stating the full address of the place where the x-ray device, which does not contain a radioactive source, is stored, and that a license application will be made in case the device is planned to be reused.

(9) A copy of the report showing that the x-ray device that does not contain a radioactive source was scrapped.

(10) The document showing that the radioactive source or the device containing the radioactive source was sent to the origin (bill of lading, etc.)

(11) A copy of the radioactive waste delivery report issued by the Radioactive Waste Processing and Storage Unit of TENMRA Nuclear Energy Research Institute
